Support Worker – Aged Care | Perth WA

Southern Cross Care WA is seeking caring and motivated Aged Care Support Workers to join their community home care team across Perth. This is a part-time role offering flexibility, meaningful work, and the chance to support older West Australians to live safely and independently in their own homes.

Please note: This is an aged care role only (not disability support).
A reliable personal vehicle is required for work purposes.

About the Role

As an Aged Care Support Worker, you will provide high-quality, person-centred care to clients in their own homes. You will spend a significant portion of your day travelling between client locations, supporting independence, wellbeing and dignity.

Southern Cross Care WA is currently seeking staff with 3–5 days weekday availability and a warm, approachable attitude.

Key Service Areas:

• Joondalup
• Fremantle
• South Perth
• Mandurah

Key Responsibilities

• Assist with daily living activities including:
  • Personal care and hygiene
  • Medication monitoring
  • Meal preparation and shopping
  • Domestic assistance and cleaning
  • Social support and companionship
• Follow individual client care plans accurately
• Record care activities using a smartphone system
• Communicate regularly with Care Coordinators
• Build strong, trusting relationships with clients and families

About You

You are compassionate, reliable and confident working independently in the community. You may already have experience in aged or community care, or you may be looking to start a rewarding career after caring for a loved one.

Essential Skills & Attributes:

• Strong communication and interpersonal skills
• Ability to prioritise tasks and multitask effectively
• Confidence using smartphones and care apps
• Initiative and a client-focused mindset

Essential & Desirable Requirements

Essential:

• Valid Australian Driver’s Licence
• Reliable, roadworthy vehicle
• Current First Aid Certificate

Desirable (but not mandatory):

• Certificate III in Aged Care / Individual Support / Community Services
• Manual Handling and/or Medication Administration certificates
• NDIS Worker Screening Check
