About the Role
As a Support Worker in Perth, you will provide high-quality, individualised home care services under CHSP, VHC and HCP programs. Your role will focus on empowering clients to maintain independence, dignity, and quality of life within their own homes.
Key Responsibilities
- Meal assistance, including shopping and meal preparation
- Transport and accompaniment to appointments and shopping
- Social support, outings and meaningful activities
- Personal care such as grooming, hygiene, bathing and dressing
- Support for health conditions, including medication reminders, skin care and diabetes support
- Mobility assistance
- Domestic assistance, including cleaning and laundry
About You
You are caring, respectful, and motivated to make a positive impact in people’s lives. You work well independently while also contributing positively to a team environment.
Essential Requirements
To be successful in this Support Worker Perth role, you will need:
- Certificate III in Aged Care / Individual Support (or similar) or equivalent experience
- Kind, respectful and person-centred approach to care
- Strong communication and interpersonal skills
- Ability to work with people from diverse cultural and social backgrounds
- Current Australian driver’s licence and reliable vehicle
- Australian working rights
- Current National Police Check (within last 6 months)
Desirable (but not essential)
- Prior experience in community or home care
- Disability or residential aged care experience
- Current First Aid Certificate
