Care Partner Perth – Job Description

The Care Partner Perth role involves managing and coordinating care for clients participating in the Support at Home Program.

The Care Partner will support clients with low, medium and high care requirements and take a holistic, person-centred approach to care management.

The position requires strong communication and relationship-building skills because the Care Partner will collaborate with clients, carers, families, General Practitioners, Allied Health Professionals, community organisations and internal clinical care providers.

There may also be an element of direct care provision required as part of the role.

Responsibilities

As a Care Partner Perth, your responsibilities may include:

  • Coordinating and leading care management activities for Support at Home clients.
  • Supporting clients with low, medium and high care requirements.
  • Conducting initial client assessments.
  • Completing ongoing assessments to evaluate client abilities.
  • Monitoring Support at Home program costs.
  • Ensuring services operate within budget requirements.
  • Maintaining high customer service standards.
  • Developing individual care plans according to client needs.
  • Completing accurate documentation.

Requirements

Applicants for the Care Partner Perth position should have a strong background in the aged care industry and recent Support at Home experience or strong knowledge of the program.

Key requirements include:

  • Certificate III qualification in Aged Care OR a minimum of 12 months Case Management experience.
  • Basic First Aid Certificate.
  • Solid Case Management experience.
